My Mazda MPV failed smog check. Would like to apply for CAP Repair Assistance.

My 2001 Mazda MPV failed the smog test at a local dealership. The dealership's smog technician found an O2 sensor that was bad and would not let it pass the smog check. I read about your program for payment of smog check related repairs and a small co-payment for people on disability. If I qualify can you send me the correct forms and let me know what I have to do.

CAP will pay the Gold Shield smog station of your choice up to $500.00 dollars in smog diagnosis and repairs through the Consumer Assistance Program (CAP).
